High School Music Club Registration

High School Club Registration

  • All tickets requested through this form will be mailed to the name and address provided. 
  • Tickets requested the week of the performance will need to be arranged through Patron Services by calling 801-533-NOTE (6683) during normal business hours.
  • Students may order tickets and attend on their own. Teachers and non-teacher adults are invited to accompany students as chaperones.
  • All Opera-tunities final dress rehearsals are scheduled Thursday evenings from 7:00-10:00pm at the Janet Quinney Lawson Capitol Theatre.
  • Tickets are required for every event. 
  • All information must be filled out in order to complete your request.

Step 6 – Order Completion

Once you click SUBMIT, a notification will be sent to USUO to be processed manually by our Patron Services team prior to your performance. Please allow 1-2 business days for completion.

If an amount is due, Patron Services will contact you by phone to collect payment.

An email confirmation will be sent to the address provided when the ticket order has been finalized. Please check your promotions or spam folders if you cannot locate the confirmation email in your inbox.
